All active volunteers are required to complete the Annual Volunteer Participation Form. Completing this quick online form helps 4-H Ontario to ensure that staff and volunteers are equipped with the information needed to facilitate 4-H activities, be able to respond in the event of an emergency and keep participants up to date on 4-H activities.
This information will be shared with your regional Volunteer Support Coordinator as well as your association’s Membership Coordinator. Please complete the form prior to participating in any volunteer activities in 2024.
