Ontario Mutuals Grant Application – Final Report

The Organization shall provide to 4-H on a written report summarizing the charitable activity engaged in using the funds of 4-H under the supervision of the Organization during the reporting period on each Project. Specifically, the report will include detailed project-specific operational reporting, including such items as photos, minutes of meetings and all other records.  We will accept pictures of events, quotes or thank you notes from participants to share with donors.

Please submit your final report by December 31, 2022.
